The requirements for issuance of special combination vehicle permits are the following:

1. (1) Where to apply. Application shall be made to the Director of the Size and Weight Permit Division, Department of Transportation, 200 N.E. 21^st Street, Oklahoma City, Oklahoma, 73105, on a form prescribed by the Department.
2. (2) Cost. A completed and signed application will be accompanied by a tender of an annual fee of Two Hundred Forty Dollars ($240.00) for each permit issued.
3. (3) Number. One permit is required for each special combination vehicle.
4. (4) Expiration. Permits will expire one year from the date of issuance.
5. (5) Refunds. Refunds for unused portions of issued permits will not be made.
6. (6) Renewal. The filing of an application for permit or renewal of same does not authorize operation. Operation may only commence after the issuance of a permit by the Department of Transportation.
