(a) Applicants for licensure as a LCSW, LMSW, LSW, LSW-Adm or LSWA shall make such application using the most current form and format prescribed by the Board, shall submit the required fee and shall supply documentation as required by the Board to validate the facts which are claimed as a part of the licensing process. Such items may include but not be limited to:
- (1) an official transcript
- (2) Photograph taken within the last 12 months
- (3) a state and national criminal history record check using background check process approved by the Board
- (4) verification of supervision, as applicable
- (5) an official score transfer from the Association of Social Work Boards, as applicable
- (6) a verification of licensure from another state, as applicable
- (7) an affidavit of legal status comfirming US citizenship or qualified legal alien status
- (b) The fee for the national criminal history record check shall be paid separately from the license application fee by the applicant.
- (c) The use of false or fraudulent information by an applicant may be grounds for denial of a license.
