The Board shall authorize the issuance of licenses to persons who qualify as follows:

1. (1) Has master's degree in social work from a Board approved social work program and has a total of 3,000 hours of supervised experience in the practice of social work under professional supervision of a licensed clinical social worker or licensed social worker with administration specialty. This experience shall include at least 100 hours of face-to-face educational supervision by the supervisor. Ratings on the final evaluations completed by supervisor(s) must document performance at a level meeting or exceeding expectations as defined on the forms approved by the Board, in all areas of the evaluation,
2. (2) Has passed the examination provided for under the provisions of Section 675:10-1-10, and
3. (3) Is of good moral character.
