- (a) The fee schedule shall be:
- (1) The fee for an original application for subscription by an employer shall be Fifty Dollars ($50.00).
- (2) The fee for a renewal subscription of an employer shall be Fifty Dollars ($50.00).
- (3) The fee for an original enrollment of a driver by an employer shall be Five Dollars ($5.00) plus the fee for a motor vehicle report as prescribed in 47 O.S. § 6-117.
- (4) The fee for a renewal enrollment of a driver by an employer shall be Five Dollars ($5.00) plus the fee for a motor vehicle report as prescribed in 47 O.S. § 6-117.
- (b) Companies will be invoiced for payment to Service Oklahoma. Payment is due upon receipt of invoice.
- (c) No fee shall be subject to refund or to prorating.
