Name change request. Any change of name of a licensee or licensed firm must be filed in the Commission office within ten (10) days of such change. Filed shall mean the date of the United States postal service postmark or the date personal or electronic delivery is made to the Commission. Upon any request for a change of name there shall be paid a fee to the Commission of Twenty-five Dollars ($25.00) for each license to be changed. The Commission may require additional documents as may reasonably be required by the Secretary-Treasurer.
