Licensed wrecker services shall complete a Tow Request and Authorization Form, in accordance with the instructions on the form, prior to removing illegally parked or abandoned vehicles from private property. Within seventy-two (72) hours of removing an illegally parked or abandoned vehicle from private property, the licensed wrecker service shall:

1. (1) Submit one (1) copy of the Tow Request and Authorization form to the Department, and:
2. (2) Submit one (1) copy of the Tow Request and Authorization form to the owner, legal possessor, or agent of the owner of the real property from which the vehicle was removed, and;
3. (3) Submit one (1) copy of the Tow Request and Authorization form to the local law enforcement agency having jurisdiction over the real property from which the vehicle was removed, and;
4. (4) Retain one (1) copy of the Tow Request and Authorization form.
