1. (a) To be treated as a complete application and to not be subject to a statutory late fee penalty, the original annual renewal application must meet all of the following standards

   1. (1) Be received by the OBPVS on or before the statutory deadline;
   2. (2) Contain an accurate school-completed OBPVS-provided Checklist;
   3. (3) Omit no more than two (02) required documents;
   4. (4) Have no more than two (02) individual documents that are incomplete by nature of an absent signature or absent notarization; and
   5. (5) Be amended within thirty (30) days of the original OBPVS receipt by submission to and OBPVS receipt of the missing item(s) or element(s) without OBPVS prompting.
2. (b) For purposes of evaluating completeness, each four (04) personnel related forms to attest to felony convictions or association with an entity with a prior adverse school action(s) shall count as a single omitted item.
