1. (a) The Oklahoma Police Pension and Retirement System shall supply the physical-medical examination form which shall be completed by the applicant and the examining medical professional and submitted to the Oklahoma Police Pension and Retirement System prior to employment with a participating municipality. The physical-medical examination form shall include a release for medical/psychological information.
2. (b) The completed physical-medical examination form shall be provided to the Oklahoma Police Pension and Retirement System for submission to the Board's reviewing physician or medical professional to identify any preexisting medical/psychological conditions.

3. (c) The physical-medical examination shall cover, but is not limited to, the following medical conditions:

   1. (1) Head and Neck.

      1. (A) Head:

         1. (i) Deformities of the skull such as depressions or exostoses.
         2. (ii) Deformities of the skull associated with evidence of disease of the brain, spinal cord, or peripheral nerves.
         3. (iii) Loss or congenital absence of the bony substance of the skull.

      2. (B) Neck:

         1. (i) Thoracic outlet syndrome.
         2. (ii) Congenital cysts, chronic draining fistulas, or similar lesion.
         3. (iii) Contracture of neck muscles.

      3. (C) Eyes and vision:

         1. (i) Far visual acuity in each eye.
         2. (ii) Peripheral vision.
         3. (iii) Diseases of the eye such as retinal detachment, progressive retinopathy, or optic neuritis.
         4. (iv) Ophthalmological procedures such as radial keratotomy or repair of retinal detachment.

      4. (D) Ears and hearing:

         1. (i) Auditory canal -- atresia, severe stenosis, or tumor.
         2. (ii) Severe external otitis.
         3. (iii) Auricle -- severe agenesis or traumatic deformity.
         4. (iv) Mastoid -- severe mastoiditis or surgical deformity.
         5. (v) Meniere's syndrome or labyrinthitis.
         6. (vi) Otitis media.
         7. (vii) Hearing deficit in the pure tone thresholds, with or without aids in both ears, in the following frequencies: 500 Hz, 1000 Hz, 2000 Hz, 3000 Hz, 4000 Hz, and 6000 Hz.

      5. (E) Dental:

         1. (i) Diseases of the jaws or associated tissues.
         2. (ii) Orthodontic appliances.
         3. (iii) Oral tissues, extensive loss.

      6. (F) Nose, oropharynx, trachea, esophagus, and larynx:

         1. (i) Tracheostomy.
         2. (ii) Aphonia.
         3. (iii) Congenital or acquired deformity.
         4. (iv) Allergic respiratory disorder.
         5. (v) Sinusitis, recurrent.
         6. (vi) Dysphonia.

   2. (2) Heart and vascular system.

      1. (A) Heart:

         1. (i) Current angina pectoris.
         2. (ii) Myocardial insufficiency.
         3. (iii) Acute pericarditis, endocarditis, or myocarditis. Chronic pericarditis, endocarditis with resultant significant valvular lesions, or myocarditis leading to myocardial insufficiency or excludable arrhythmias.
         4. (iv) History of myocardial infarction, coronary artery bypass, or coronary angioplasty.
         5. (v) Cardiac pacemaker.
         6. (vi) Recurrent syncope.
         7. (vii) Significant valvular lesions of the heart including prosthetic valves.
         8. (viii) Coronary artery disease.
         9. (ix) Atrial tachycardia, flutter, or fibrillation.
         10. (x) Third degree atrio-ventricular block.
         11. (xi) Ventricular tachycardia.
         12. (xii) Hypertrophy of the heart.
         13. (xiii) Recurrent paroxysmal tachycardia.
         14. (xiv) History of a congenital abnormality.

      2. (B) Vascular system:

         1. (i) Congenital or acquired lesions of the aorta and major vessels.
         2. (ii) Marked circulatory instability as indicated by orthostatic hypotension, persistent tachycardia, and severe peripheral vasomotor disturbances.
         3. (iii) Aneurysm of the heart or major vessels, congenital or acquired.
         4. (iv) Hypertension.
         5. (v) Peripheral vascular disease such as Raynaud's phenomenon.
         6. (vi) Recurrent thrombophlebitis.
         7. (vii) Chronic lymphedema due to lymphopathy or severe venous valvular incompetency.

   3. (3) Lungs, abdomen, spine and joint.

      1. (A) Lungs and chest wall:

         1. (i) Suppurative disease of lung or pleural space.
         2. (ii) Lobectomy.
         3. (iii) Bronchial asthma.
         4. (iv) History of bronchiectasis, bronchitis, fibrous pleuritis, fibrosis, cystic disease, tuberculosis, or mycotic disease of the lung.
         5. (v) Pneumothorax.
         6. (vi) Restrictive or obstructive lung disease.

      2. (B) Abdominal organs and gastrointestinal system:

         1. (i) Cholecystectomy or cholecystitis.
         2. (ii) Gastritis.
         3. (iii) Hemorrhoids.
         4. (iv) Acute hepatitis.
         5. (v) Hepatitis B.
         6. (vi) Hepatitis C.
         7. (vii) Hernia.
         8. (viii) Inflammatory bowel disease.
         9. (ix) Intestinal obstruction.
         10. (x) Pancreatitis.
         11. (xi) Resection, bowel.
         12. (xii) Ulcer, gastrointestinal.
         13. (xiii) Cirrhosis, hepatic or biliary.

      3. (C) Spine, scapula, ribs, and sacroiliac joints:

         1. (i) Arthritis.
         2. (ii) Structural abnormality, fracture, or dislocation.
         3. (iii) Nucleus pulposus, herniation of or history of laminectomy.

      4. (D) Extremities:

         1. (i) Limitation of motion of a joint.
         2. (ii) Amputation or deformity of a joint or limb.
         3. (iii) Dislocation of a joint.
         4. (iv) Joint reconstruction, ligamentous instability, or joint replacement.
         5. (v) Chronic osteoarthritis or traumatic arthritis.
         6. (vi) Inflammatory arthritis.

   4. (4) Genitourinary system.

      1. (A) Reproductive:

         1. (i) Pregnancy.
         2. (ii) Dysmenorrhea.
         3. (iii) Endometriosis, ovarian cysts, or other gynecologic conditions.
         4. (iv) Testicular or epididymal mass.

      2. (B) Urinary system:

         1. (i) Diseases of the kidney.
         2. (ii) Diseases of the ureters, bladder, or prostate.

   5. (5) Other conditions.

      1. (A) Neurological disorders:

         1. (i) Ataxias of heredo-degenerative type.
         2. (ii) Cerebral arteriosclerosis as evidenced by documented episodes of neurological impairment.
         3. (iii) Multiple sclerosis with activity or evidence of progression within previous three years.
         4. (iv) Progressive muscular dystrophy or atrophy.
         5. (v) Any form of seizure disorder [simple partial, complex, generalized, psychomotor or absence (petit mal)].
         6. (vi) Narcolepsy.
         7. (vii) Congenital malformations.
         8. (viii) Migraine.
         9. (ix) Clinical disorders with paresis, paralysis, dyscoordination, deformity, abnormal motor activity, abnormality of sensation, or complaint of pain.
         10. (x) Subarachnoid or intracerebral hemorrhage.
         11. (xi) Abnormalities from recent head injury such as severe cerebral contusion or concussion.

      2. (B) Skin:

         1. (i) Acne or inflammatory skin disease.
         2. (ii) Eczema.

      3. (C) Blood and blood-forming organs:

         1. (i) Hemorrhagic states requiring replacement therapy.
         2. (ii) Sickle cell disease (homozygous).
         3. (iii) Anemia.
         4. (iv) Leukopenia.
         5. (v) Polycythemia vera.
         6. (vi) Splenomegaly.
         7. (vii) History of thromboembolic disease.

      4. (D) Endocrine and metabolic disorders:

         1. (i) Diseases of the adrenal gland, pituitary gland, parathyroid gland, or thyroid gland of clinical significance.
         2. (ii) Nutritional deficiency disease or metabolic disorder.
         3. (iii) Diabetes mellitus.

      5. (E) Systemic diseases and miscellaneous conditions:

         1. (i) Connective tissue disease, such as dermatomyositis, lupus erythematosus, scleroderma and rheumatoid arthritis.
         2. (ii) Residuals from past thermal injury.
         3. (iii) Documented evidence of a predisposition to heat stress with recurrent episodes or resulting residual injury.
      6. (F) Tumors and malignant diseases.

      7. (G) Psychiatric conditions:

         1. (i) History of psychiatric condition.
         2. (ii) Substance abuse problems.

      8. (H) Chemicals, drugs, and medications:

         1. (i) Anticoagulant agents.
         2. (ii) Cardiovascular agents.
         3. (iii) Narcotics.
         4. (iv) Sedative-hypnotics.
         5. (v) Stimulants.
         6. (vi) Psychoactive agents.
         7. (vii) Steroids.

      9. (I) Immunologic deficiency diseases:

         1. (i) Acquired immunodeficiency syndrome (Aids).
         2. (ii) HIV positive without evidence of HIV infection.
4. (d) Medical examination results are valid for no more than six (6) months after date of examination.
5. (e) A medical examination is required for all persons who have ceased employment from a participating municipality as a police officer for more than 90 days.
