1. (a) Interview. An interview with a representative of the Commission may be required before an application is approved for a scholarship recipient.

2. (b) Application form. . The application form consists of:

   1. (1) General information.
   2. (2) Statement regarding financial needs.
   3. (3) Statement of professional goals.

3. (c) Supporting information. The following supporting application information may be required:

   1. (1) Certification of unconditional acceptance or promotion, whichever is applicable forthe application.
   2. (2) A letter certifying unconditional acceptance and/or being a student in good standingby nursing school director.
   3. (3) A letter or transcript showing high school GPA or GED score for LPN students;
   4. (4) A letter or transcript showing ACT score, college and/or high school GPA for ADN,BSN, or MSN students;
   5. (5) Current Federal Income Tax form 1040, 1040A, or 1040EZ or proper certification ifit is not necessary to file Federal Income Tax form. Dependent applicants mustprovide parent's income tax form in addition to their own form.
4. (d) Date due. The application form and supporting information will not be accepted after the applicable due date set by the Commission.
