Documents for receiving anatomical specimens

The following documents, or information enabling the Board or the Board's Agent to obtain documents, shall be provided to the Board or the Board's Agent with each anatomical specimen:

(1) Death certificate
(2) Copy of body donation form (if applicable)
(3) Cremation permits
