An application for a permit to conduct surface coal mining and reclamation operations shall be complete and shall include at a minimum:

1. (1) For surface mining activities, the information required under Subchapters 23, 25, and 27 of this Chapter, and, as applicable to the operation, Subchapter 33 of this Chapter; and
2. (2) For underground mining activities, the information required under Subchapters 23, 29, and 31 of this Chapter, and, as applicable to the operation, Subchapter 33 of this Chapter.
