1. (a) The OTP shall have written policy and procedure stating each person served accepted for treatment at an OTP shall be assessed no less than annually by the medical director or an appropriately trained program physician as part of a process to determine the most appropriate combination of services and treatment.

2. (b) Compliance with 450:70-6-15.2 may be determined by:

   1. (1) A review of policies and procedures,
   2. (2) Treatment records, and
   3. (3) Other facility documentation.
