1. (a) A periodic physical examination shall occur no less than one time each year and be conducted by an OTP practitioner. The periodic physical examination shall be documented in the clinical record and include:
2. (1) A review of MOUD dosing;
3. (2) Treatment response;
4. (3) Other substance use disorder treatment needs; and
5. (4) Other relevant physical and psychiatric treatment needs and goals.

6. (b) Written policies and procedures governing the intake and assessment process shall specify the following:

   1. (1) The information to be obtained on all applicants or referrals for admission;
   2. (2) The procedures for accepting referrals from outside agencies or organizations;
   3. (3) The records to be kept on all applicants;
   4. (4) Any prospective data regarding the person served to be recorded during the intake process;
   5. (5) The procedures to be followed when an applicant or a referral is found ineligible for admission; and
   6. (6) The procedures and policies for the purpose of admitting and assessing persons with special needs or disabilities.

7. (c) Compliance with 450:70-3-5.16 may be determined by:

   1. (1) A review of policies and procedures,
   2. (2) Treatment records, and
   3. (3) Other facility documentation.
