1. (a) Written approval from a parent or guardian for a person served under the age of eighteen (18) admitted to maintenance treatment shall be obtained if required by law. If a minor is eligible to self-consent to treatment pursuant to state law, written approval from a parent or guardian is not required.

2. (b) Compliance with 450:70-3-5.5 may be determined by:

   1. (1) A review of policies and procedures,
   2. (2) treatment records, and
   3. (3) Other facility documentation.
