1. (a) The OTP shall have written policy and procedure stating the clinical record shall contain adequate documentation of any prescription medication, including methadone or buprenorphine, that a person served may be taking, including the name of the medication, the prescription number, the dose, the reason for prescribing, the name of the prescribing doctor, the pharmacy's name and telephone number, the date it was prescribed, and the length of time the person served is to be taking the medication. A release of information to the prescribing physician either by mail, facsimile or other acceptable electronic means allowing the medical director to coordinate treatment and discuss medications.

2. (b) Compliance with 450:70-3-5.3 may be determined by:

   1. (1) A review of policies and procedures,
   2. (2) Treatment records, and
   3. (3) Other facility documentation.
