1. (a) Discharge from a CGAT program shall be determined by the following:

   1. (1) Discharge assessment to determine achievement of consumer's treatment goals and consumer's continued need for treatment services;
   2. (2) Reduction in problem gambling behaviors for at least 30 days prior to discharge; and
   3. (3) Completion of a Continuing Care Plan with linkage to community gambling support groups, or other community services.

2. (b) Compliance may be determined by a review of the following:

   1. (1) Policy and procedures;
   2. (2) Continuing Care Plans;
   3. (3) Discharge assessments;
   4. (4) Discharge summaries;
   5. (5) Progress notes;
   6. (6) Consumer Records:
   7. (7) Interviews with staff and consumers; and
   8. (8) Other facility documentation.
