When addressing issues related to the ADSAC course or assessment process that must be reflected by written documentation in the participant's record, the following must be included:

1. (1) date;
2. (2) start and stop time for each session referenced;
3. (3) signature of the staff person providing the service;
4. (4) credentials of the staff person providing the service;
5. (5) participant response if present;
6. (6) any problems identified; and
7. (7) any interventions.
