The investigator shall gather and review relevant documents including, but not limited to:

1. (1) incident reports and other written reports, accounts, and statements prepared during the preliminary assessment;
2. (2) psychiatric and medical records;
3. (3) photos; and
4. (4) facility or provider logs, activity and tracking documents.
