Each consumer shall be informed of the following:

1. (1) Present and future use and disposition of products of special observation and audiovisual techniques such as tape recorders, television, movies and photographs in which he or she voluntarily participated;
2. (2) The right to refuse to participate in any research project;
3. (3) The costs, itemized when possible, of services rendered to the consumer, the source of the facility's reimbursement and any limitation placed on duration of services;
4. (4) Right to access and view all information held by ODMHSAS and which is subject to the Open Records Act .
