1. (a) The following violations of Title 3A of the Oklahoma Statutes shall be cause for mandatory revocation:

   1. (1) 3A - 406A(1)
   2. (2) 3A - 406A(2)
   3. (3) 3A - 406B
   4. (4) 3A - 406C
   5. (5) 3A - 407A(1)
   6. (6) 3A - 407A(2)
   7. (7) 3A - 407A(3)
   8. (8) 3A - 407B(3)
   9. (9) 3A - 426

2. (b) All other violations of Title 3A of the Oklahoma Statutes shall be subject to a maximum penalty of:

   1. (1) A thirty (30) day license suspension and a $3,000.00 fine for the first violation;
   2. (2) A sixty (60) day license suspension and a $6,000.00 fine for a second violation;
   3. (3) A ninety (90) day license suspension and a $9,000.00 fine for a third violation; and
   4. (4) Revocation of license for a fourth violation.
