Licenses issued pursuant to Sections 442:10-2-1, 2, 3, and 4 of this Subchapter shall contain the following:

1. (1) The digital photograph of the license holder;
2. (2) The name and date of birth of the license holder;
3. (3) The name of parent(s) or legal guardian(s) of minor license holder, if applicable;
4. (4) The city and county of residence of the license holder;
5. (5) The type of license;
6. (6) The date the license expires; and
7. (7) The unique 24- character license number assigned to the license holder and caregiver, if applicable.
