An individual proceeding may be initiated by the filing of a request for hearing containing a brief statement setting forth the relief requested and the grounds therefor upon a form approved by the Board of Juvenile Affairs. The hearing shall be open to the public and shall be conducted in accordancewith the Administrative Procedures Act.
