1. (a) All juveniles shall receive medical clearance from the health authority prior to participation in the RJTP. All findings shall be recorded on a form approved by the health authority.

2. (b) The RJTP shall require medical, dental, and mental health screening to be performed by health-trained or qualified health care personnel on all juveniles, excluding juveniles who have had such screenings within 30 days prior to arrival. All findings must be recorded on a form approved by the health authority. The form includes:

   1. (1) inquiry into:

      1. (A) current illness and health problems, including venereal diseases and other infectious diseases;
      2. (B) dental problems;
      3. (C) mental health problems;
      4. (D) use of alcohol and other drugs, which includes types of drugs used, mode of use, amounts used, frequency used, date or time of last use, and a history of problems that may have occurred after ceasing use;
      5. (E) past and present treatment or hospitalization for mental disturbance or suicide attempt; and
      6. (F) other health problems designated by the responsible physician;

   2. (2) observation of:

      1. (A) behavior, which includes state of consciousness, mental status, appearance, conduct, tremor, and sweating;
      2. (B) body deformities, ease of movement, etc.; and
      3. (C) condition of skin, including trauma markings, bruises, lesions, jaundice, rashes and infestations, and needle marks or other indications of drug abuse; and

   3. (3) medical disposition of juvenile to the:

      1. (A) general population;
      2. (B) general population with appropriate referral to health-care service; or
      3. (C) appropriate health-care service for emergency treatment.

3. (c) All policies related to health care and mental health care shall:

   1. (1) be reviewed at least annually by the health care authority and revised, if necessary; and
   2. (2) bear the date of the most recent review or revision and signature of the reviewer.
