1. (a) Upon receipt of any surplus property donated to a Receiving Agency by the OSBI, the head of the Receiving Agency or a designee shall sign a receipt identifying the surplus property donated in sufficient detail as to be able to identify the property in the inventory of OSBI property.

2. (b) At a minimum, the receipt should include the:

   1. (1) name and address of the Receiving Agency;
   2. (2) name of the Receiving Agency head and any designee signing the receipt in question;
   3. (3) make/model, VIN or serial number or other identifying characteristics of the property;
   4. (4) OSBI inventory number of the property if applicable;
   5. (5) date of the transfer of property
   6. (6) general condition of the property.
