- (a) The disclosure required by Section 6055(B)(2)(f) of Title 36 shall be in writing on a document no smaller than five inches by seven inches (5" X 7"), in type no smaller than twelve point and posted in easy view of the public.
- (b) When any referral is made and disclosure is required pursuant to the Oklahoma Health Care Freedom of Choice Act, that disclosure shall be in writing within a document with print no smaller than eight point delivered to the insured prior to point of service.
