1. (a) Purpose. The purpose of this section is to set forth information which must be included on an insurance policy.

2. (b) Insurer's name and address required on policy.

   1. (1) 36 O.S. §3613 (1981), entitled "Contents of Policies in General," provides that each contract of insurance shall set forth the insurer's name and complete address [36:3613(b) (2)].

   2. (2) Every policy of insurance issued or delivered in this State shall specify, on its face, or by endorsement, or rider attached to the policy:

      1. (A) The complete name of the insurer;
      2. (B) The complete street address of the home or principal office of the insurer.
