1. (a) A hearing request must be in writing and signed by the appealing party.

2. (b) To be considered timely, a hearing request must be received by the Oklahoma Department of Human Services within:

   1. (1) 30-calendar days of receipt of the notice of denial or revocation;
   2. (2) 10-calendar days of receipt of an Emergency Order; or
   3. (3) 30-calendar days of receipt of the notice of recording on the Child Care Restricted Registry.
