1. (a) When a patron makes a complaint regarding the pari-mutuel department to an organization licensee, the organization licensee shall immediately issue a complaint report, setting out:

   1. (1) the name of the complainant;
   2. (2) the nature of the complaint;
   3. (3) the name of the persons, if any, against whom the complaint was made;
   4. (4) the date of the complaint;
   5. (5) the action taken or proposed to be taken, if any, by the organization licensee.
2. (b) The organization licensee shall submit every complaint report to the Commission within forty-eight (48) hours after the complaint was made.
