1. (a) Physical therapists.

   1. (1) Must be appropriately licensed in good standing in the state in which they practice and working in accordance with the Oklahoma Physical Therapy Practice Act or other applicable statute(s); and
   2. (2) Entered into a provider agreement with the Oklahoma Health Care Authority (OHCA) to provide physical therapy services.

2. (b) Physical therapist assistants.

   1. (1) Must be working under the supervision of a fully licensed physical therapist;
   2. (2) Must be appropriately licensed in good standing in the state in which they practice and working in accordance with the Oklahoma Physical Therapy Practice Act or other applicable statute(s);
   3. (3) Entered into a provider agreement with the OHCA to provide physical therapy services; and
   4. (4) Provided the name of their OHCA-contracted supervising physical therapist upon enrollment.
