The Department will compile custom data sets (CDS) based on the data elements contained in the PUDF.

1. (1) Requests for CDS must be made in writing to the Department using the Special Request Form.
2. (2) Entities requesting custom data sets from the PUDF must sign and complete the Data Use Agreement. The signed Data Use Agreement must be included with the request.
3. (3) The application fee must be received by the Department with the request.
