1. (a) An employer shall report to the Department any allegation of client or resident abuse, neglect, mistreatment or misappropriation of client's or resident's property against the employer's nurse aide.
2. (b) An employer shall report to the Department by telephone within twenty-four (24) hours after receiving an allegation and in writing within five (5) working days after receiving an allegation.

3. (c) The written report filed by the employer shall include:

   1. (1) The allegation;
   2. (2) Name and identification number of the nurse aide;
   3. (3) Date of the occurrence;
   4. (4) Results of any internal investigation;
   5. (5) Any corrective action taken by the employer; and
   6. (6) Name and address of any person who may have witnessed the incident.
