The following elements shall be provided:

1. (1) Entrance at grade level sheltered from the weather and able to accommodate wheelchairs.

2. (2) Lobby. It shall include:

   1. (A) Reception and information counter or desk.
   2. (B) Waiting space(s).
   3. (C) Public toilet facilities.
   4. (D) Public telephone(s).
   5. (E) Drinking fountain(s).
3. (3) General or individual office(s) for business transactions, private interviews, medical and financial records, and administrative and professional staff
4. (4) Multipurpose room for conferences, meetings, and health education purposes including facilities for showing visual aids.
5. (5) Storage for office equipment and supplies.
