1. (a) The approved program shall maintain participant records for at least three (3) years which shall include, but not be limited to, the following:

   1. (1) Attendance records; and
   2. (2) Results of examination of course curriculum components.

2. (b) The approved program shall submit a report to the Department within fourteen (14) days of completion of each program which shall include, but is not limited to, the following:

   1. (1) The name of the institution providing the program;
   2. (2) The month and year of program completion; and
   3. (3) An alphabetical list of participants who successfully completed the course and who are eligible to take the OHCAPA.
