1. (a) The hospice must develop, maintain, and conduct a comprehensive quality assurance program that includes an evaluation of services, quarterly clinical record audits, and organizational review.
2. (b) The hospice must ensure that appropriate and quality care is provided to include inpatient care, home care, and care provided under arrangements.
3. (c) The quality assurance program must be reviewed at least once a year. Policies and procedures will be revised as needed, reviewed, and approved annually. Goals will be established and problems identified with documented results.
