1. (a) Medical records must be kept confidential. Only authorized personnel have access to the record. Written consent of the patient, patient representative, the court appointed guardian or a court order will be accepted as authority for release of medical information.
2. (b) An individual who is, or has been, a patient of a physician, hospital, or other medical facility, except psychiatric, is entitled to access information contained in the individual's own medical records upon request. A request for minors may be made by parents or legal guardian. The hospice must furnish a copy of the medical record upon payment for the charge of such copy.
