1. (a) By May 1 of each year, the Plan shall file:

   1. (1) Financial statements which exhibit the Plan's financial condition on December 31 of the prior year and its income and expenses that year. These statements shall be prepared in accordance with accounting principles generally accepted in the United States of America. The Plan shall supplement the statements with any information requested by the Commissioner. Such statements shall be subscribed and sworn to by the president, secretary and other officers, with original signatures. A statement of financial condition shall include:

      1. (A) A balance sheet; and
      2. (B) Revenues and expenses;
   2. (2) Summaries of the grievance and dispute resolution activities in the preceding year.

2. (b) By May 1 of each year, the Plan shall report:

   1. (1) Service volume for the prior year;
   2. (2) Results of the quality assurance procedures;
   3. (3) Providers as of December 31 of the prior year; and
   4. (4) Number of employees by zip code of residence or work site as of December 31 of the prior year.
