1. (a) The proposal shall be submitted using the forms provided by the Department. The proposal form shall include the following sections:

   1. (1) Proposal Information, including the name of the contact person, mailing address, e-mail address, phone number and type of qualifying applicant entity.
   2. (2) Instructions, including an outline of the legal requirements and the priority point system.
   3. (3) A section requiring a narrative description of the proposed project.
   4. (4) A section enumerating the requirements of the OERSSIRF statute, requiring a description of the proposed project's compliance with each section.
   5. (5) A section requiring a narrative description of the proposed project's compliance with each of the priority point criteria.
   6. (6) A checklist allowing evaluation of compliance with solicitation requirements.

2. (b) Each proposal shall include a section setting forth the criteria that will be used to evaluate the success of the project. The criteria shall include:

   1. (1) Specific, objective metrics for evaluation of the project. For example: a percentage decline in response time or improvement in the number of available EMTs within a region, measured against the same metric at the start of the project.
   2. (2) A clear methodology and a description of data sources for computing the performance measures proposed in the project plan, for example, comparing responder response times or the total number of EMTs in a region against the same metric at the end of the project.

   3. (3) Benchmark measures for each of the following assessment levels:

      1. (A) Significantly improved.
      2. (B) Improved.
      3. (C) Not Improved.
      4. (D) Worsened.

<i>¹This emergency action expired before being superseded by a permanent action. Upon expiration of an emergency action enacting a new Section, the text of the Section is no longer effective.
