Requirements for renewal

Requirements for renewal include the following:

(1) Compliance with the Act and this Chapter;
(2) Documentation of current certifications listed in OAC 310:395-7-2 of this Chapter with the exceptions of (1) and (7); and
(3) Payment of the renewal fee(s).
