1. (a) Each application shall include the following documents:

   1. (1) Application form;
   2. (2) Official documentation showing the applicant meets the requirements listed in OAC 310:395-7-2; and
   3. (3) Fee(s).

2. (b) The application form requires the following:

   1. (1) Identifying information;
   2. (2) Resume with relevant work history;
   3. (3) Possession of other credentials;
   4. (4) Previous misconduct or disciplinary actions;
   5. (5) Declaration of previous court judgements against the applicant related to midwifery care; and
   6. (6) Other information that may be required by the Department.
