If an application for a license to operate is denied, the regulatory authority shall provide the applicant with a notice that includes:

1. (1) The specific reasons and Chapter citations for the license denial;
2. (2) The actions, if any, that the applicant must take to qualify for a license; and
3. (3) Advisement of the applicant's right of appeal.
