1. (a) Tattoo or body piercing artist license and registration fees are as follows:

   1. (1) Student: $0;
   2. (2) Sponsor: $0;
   3. (3) Apprentice: $250.00;
   4. (4) Initial: $250.00;
   5. (5) Renewal: $250.00;
   6. (6) Renewal thirty (30) days after expiration: $350.00; and

   7. (7) Temporary: $50.00;

      1. (A) Not to exceed seven (7) consecutive days; and
      2. (B) Not to exceed thirty (30) total days per year.

2. (b) Establishment license fees are as follows:

   1. (1) Tattoo:

      1. (A) Initial: $1,000.00.
      2. (B) Renewal: $500.00.
      3. (C) Renewal thirty (30) days after expiration: $750.00.
      4. (D) Event: $500.00.

   2. (2) Body piercing:

      1. (A) Initial: $500.00.
      2. (B) Renewal: $250.00.
      3. (C) Renewal thirty (30) days after expiration: $350.00.
      4. (D) Event: $250.00.
