1. (a) The conditions of the Federalwide Assurance apply whenever:

   1. (1) the OSDH IRB provides review and oversight of federally- supported human subject research, regardless of where the research takes place or by whom it is conducted; or
   2. (2) the OSDH becomes engaged in federally supported human subject research.

2. (b) The OSDH becomes so engaged whenever:

   1. (1) OSDH employees or agents intervene or interact with living individuals for purposes of federally supported research;
   2. (2) OSDH employees or agents obtain, release, or access individually identifiable private information for purposes of federally-supported research; or
   3. (3) The OSDH receives a direct federal award to conduct human subject research, directly or where all activities involving human subjects are carried out by a subcontractor or collaborator.
3. (c) Information provided under the Federalwide Assurance will be updated on schedule in order to maintain an active Assurance.
