1. (a) Employees may elect to enroll in this benefit during the Option Period prior to each Plan Year.
2. (b) New Hires may elect to participate in this optional benefit when completing the new hire benefits election form within thirty (30) days following date of hire.
3. (c) Requests for enrollment must be made in a manner and on forms prescribed by the Plan Administrator.
