Owners of waterworks or wastewater works shall:

1. (1) upon request, furnish DEQ with the names, addresses, and positions of all employees;
2. (2) advise DEQ in writing within ten (10) days when an operator begins or terminates employment;
3. (3) furnish the necessary equipment and materials for adequate maintenance and operation of the treatment plant, laboratory, and supporting facilities;
4. (4) give operators a reasonable opportunity to obtain the necessary hours of training for required certification upgrades and renewals; and

5. (5) within ten (10) days after contracting with an associated laboratory for the analysis of its water quality samples, notify DEQ in writing about whether:

   1. (A) the laboratory is currently certified by DEQ; and
   2. (B) the laboratory will only be performing operational testing.
