In addition to the requirements of OAC 252:653-7-2, permit applications for the construction of infiltration basins shall include the following:

1. (1) construction details to include:

   1. (A) number of infiltration basin(s);

   2. (B) dimensions of the infiltration basin(s), including:

      1. (i) surface area in acres;
      2. (ii) depth and horizontal extent of trenches or other excavations to increase recharge;
      3. (ii) maximum depth of water in basin; and
      4. (iii) freeboard allowance
   3. (C) gradients and elevations;
   4. (D) method of construction;

   5. (E) materials used;

      1. (i) soil type and permeability;
      2. (ii) filter fabric; and
      3. (iii) aggregate material
   6. (F) erosion control measures;
   7. (G) flood control features;
   8. (H) inflow and outflow/overflow points;
   9. (I) measurement points; and
   10. (J) monitoring equipment.

2. (2) operating parameters:

   1. (A) schedule of hydraulic loading and basin drying cycles;
   2. (B) estimated flow rate;
   3. (C) estimated infiltration rate for each basin;
   4. (D) estimated hydraulic loading rate with consideration given to drying time; and
   5. (E) distance to maintain between bottom of basin and top of groundwater mound
3. (3) closure plan
4. (4) any additional information DEQ determines to be necessary for the specific ASR project.
