The DEQ shall begin proceedings to summarily suspend or revoke the permit for failure to:

1. (1) establish financial assurance in accordance with this Subchapter;
2. (2) renew or maintain an approved financial assurance mechanism as required; or
3. (3) provide acceptable substitute financial assurance when necessary.
