Fees

(a) Initial accreditation. Application shall be accompanied by fees established in the following schedule:
1. (1) Per course.
  1. (A) Filing and review fee, $400.00
  2. (B) Accreditation fee, $50.00.
2. (2) Per training program.
  1. (A) Training Manager Approval Fee, $100.00.
  2. (B) Principal Instructor Approval Fee, $50.00.
3. (3) Other State/Agency Training Program Recognition & Approval. In addition to "Per Course" and "Per Training Program" fees:
  1. (A) Equivalency Review Fee, $2000.00.
  2. (B) Recognition & Approval Fee (In place of Accreditation Fee), $250.00.
(b) Renewal accreditation. Application shall be accompanied by fees established in the following schedule:
1. (1) Per course.
  1. (A) Filing and review fee, $200.00.
  2. (B) Accreditation fee, $50.00.
2. (2) Per training program.
  1. (A) Training Manager Approval Fee, $50.00.
  2. (B) Principal Instructor Approval Fee, $25.00.
3. (3) Other State/Agency Training Program Recognition & Approval. In addition to "Per Course" and "Per Training Program" fees:
  1. (A) Equivalency Review Fee, $1000.00.
  2. (B) Recognition & Approval Fee (In place of Accreditation Fee), $125.00.
4. (4) Renewal late fee. $750.00.
