To receive a job search allowance, the applicant must provide the following information for each scheduled interview attended:

1. (1) Name of the company;
2. (2) Address of the company;
3. (3) Name of the contact person;
4. (4) Telephone number of the contact person; and,
5. (5) Date of the interview.
