1. (a) Incubator sponsors shall file annual reports with the Department not later than December 15 of each calendar year.

   1. (1) Each report must contain the following information:

      1. (A) Number of new incubator tenants by industry for current reporting period;
      2. (B) Total cumulative number of incubator tenants by industry;
      3. (C) Total number of new jobs created by tenants for current reporting period;
      4. (D) Total cumulative number of jobs created by tenants;
      5. (E) Total financial value of initial investment by tenants for current reporting period;
      6. (F) Total financial value of cumulative investment by tenants; and
      7. (G) Number of firms still operating in Oklahoma after ending their tenancy in the incubator, and the number of jobs provided by these firms for the current reporting period; and
   2. (2) Failure to submit annual reports as requested herein may result in decertification of the incubator facility.

2. (b) The Department, on or before December 31 of each year, shall provide a report to the Speaker of the House of Representatives and the President Pro Tempore of the Senate containing the following information:

   1. (1) The number of applications for incubators submitted;
   2. (2) The number of applications approved;
   3. (3) The number of incubators created under this act;
   4. (4) The number of tenants occupying each incubator;
   5. (5) The number of jobs provided by each incubator and tenants of each incubator; and
   6. (6) The number of firms still operating in the State after ending their tenancy in incubators, and the number of jobs they have provided.
