Any of the following circumstances indicates a violation of program procedures and policies:
- (1) Notice by the Department that violations have resulted in substantial non-compliance or non-conformance with regard to the funded contract of a substate planning district; or
- (2) Knowingly providing false information in the request for qualification process or in project reports; or
- (3) Failure to comply with federal or state statutes or the rules of the department; or
- (4) Significant deviation from the grant contract; or
- (5) Expenditure of State Appropriated Funds project funds in a contractually prohibited or unlawful manner.
